Hemodynamic Changes of Hypertensive Patients during Perioperative Period


Abstract
One of the anesthesiologist's daily practices involves the care of hypertensive patients. Circulatory instability occurs frequently in hypertensive patients during perioperative period.
In the perioperative period, hypertensive patients of ten experience the episodes of marked hypertersion or hypotension, myocardial ischemia and arrhythmias. Therefore, it is very important to control the depth of anesthesia to minimize the wide
fluctuation of blood pressure during perioperative period.
The author classified the hypertensive patients into systolic, diastolic, and combined (systolic and diastolic) hypertension to confirm whether there were significant differences of blood pressure fluctuation among the groups or not. The author
investigated the changes of blood pressure and heart rate during perioperative period on 32 normotensive patients(normotensive group) and 63 essential hypertensive patients(hypertensive group), divided into ghree subgroups such as systolic and
diastolic
hypertensive group(26 patients), isolated systolic hypertensive group(18 patients) and isolated diastolic hypertensive group(19 patients) at Chung-Ang university Hospital from May 1992 to February 1993.

@EN 1. The incidence of use of antihypertensive was 3% in normotensive group but 46% in systolic and diastolic hypertensive group 33% in isolated systolic hypertensive group and 21% in isolated diastolic hypertensive group during perioperative
period.
2. The most common preoperative chest X-ray and EKG finding was within normal limit in the three hypertensive groups.
3. Systolic and diastolic blood pressure were more fluctuated in systolic and diastolic hypertensive group than isolated systolic hypertensive group, and isolated diastolic hypertensive group and similar fluctuation to normotensive group during
perioperative period.
4. Heart rate in systolic and diastolic hypertensive group had similar fluctuation with normotensive group while isolated systolic and isolated diastolic hypertensive group had little fluctuation during perioperative period.
